Google Chrome on an Apple Operating System
In order to save the information you have filled in and send it to us, you must have chosen "Download PDFs" in Chrome's settings.

To do this:
- Log into your Google Account
- At the top right, click on the 3 vertical grey dots
- Choose Settings from the menu dropdown
- Scroll down to Privacy and security and click on Site Settings
- Scroll to the bottom and click on Additional content settings
- Click on PDF documents and turn on Download PDF files instead of automatically opening them in Chrome
If you use Acrobat Reader, please send us the filled in form by clicking on the envelope icon in the upper right and then clicking on the Next button - DO NOT turn on Attach a Link. You will then be able to send the email to us.
If you use Preview, click on File, Share, Mail. You will then be able to send the email to us.
