Forms you need for your first Appointment
We ask that all new patients fill out the Patient Intake Form before arriving at the first appointment.
Although the form is designed as a fillable PDF, it does not work consistently with all browsers. We have created some emailing instructions once the form has been filled out (see below).
Please also note that the fillable pdf does not work when accessing with a browser on an iPad.
If you are not able to fill in the form on-line, please print the form and hand-write in the information. You would then need to scan or photo the finished form and email it back to us. We apologize for any inconvenience this may cause.

Any questions that you have will be answered by our Certified Orthotist when he reviews the Health Consent Form with you. He will also ensure that you understand our warranty policies as outlined in the Welcome To booklet.

Limited testing with Android shows that the PDF will work using an external editor such as Adobe Acrobat Reader.
